vehicle incident form

Hope everyone out there is safe and well.
Question: I am looking at revamping my vehicle accident form (which i have 2 but looking to combine), does anyone have such a form for employees to complete that they are willing to share with me.
we had our own forms, but recently changed insurer - we now use theirs (otherwise we ended up duplicating the info anyway).
You could talk to your insurer and see f they have a standard template?

then I can send you our accident card that is available in our vehicle glove box' to record the detail at the time of the accident.
As Stephen said you complete a form, then you complete the insurer's claim form and then process the incident in your own management system. So if you can have the accident form from the insurance company readily available then you completed that and process that in your own management system.
We use the insurers form and load it against the event in the H&S system, no need to do our own form.
