    In the Mar/Apr edition of Safeguard magazine we take a look at the Cave Creek platform collapse disaster, which happened 25 years ago this month.

    The Commission of Inquiry found - among many other things - that the platform had been poorly built by DoC staff with little or no construction experience.

    In your experience, is this kind of DIY approach now a thing of the past in our workplaces?
